MATÉRIEL ET MÉTHODE : L’ensemble des cas histologiques de MAT, à l’exception de ceux sur greffon rénal, répertoriés au CHU de Rouen, entre 1995 et 2015, ont été relus par deux anatomopathologistes. Le but de cette étude est de déterminer la prévalence et la localisation du C4d, dans les reins natifs atteints de MAT et également de rechercher d’éventuelles corrélations anatomo-cliniques entre les différentes étiologies de MAT. L’émergence de thérapies ciblant la voie terminale du complément incite à trouver des biomarqueurs de l’activation du complément. Le C4d est un produit inactif de dégradation secondaire à l’activation du complément. L’activation du complément a été démontrée comme jouant un rôle clé dans certaines étiologies de MAT. La MAT est une affection rare, touchant fréquemment les reins et qui se caractérise par la formation de thrombi vasculaires, d’une hémolyse et d’une thrombocytopénie. Le système du complément est un acteur clé de l’immunité innée, nécessaire à la défense de l’hôte contre les agents pathogènes mais aussi impliqué dans de diverses pathologies rénales, dont les microangiopathies thrombotiques (MAT). Future studies should correlate C4d staining patterns with circulating donor-specific antibodies. In addition, we determined the inter-observer concordance in 240 paraffin-embedded biopsies of a multi-centre study. We compared the staining extent (diffuse, focal, minimal, no staining) in frozen and paraffin sections and evaluated the intra- and inter-observer concordance rates using kappa statistics. We investigated the expression of C4d along PTC in 64 renal allograft biopsies using a monoclonal antibody (Quidel) and immunofluorescence for frozen (F-IF) and a polyclonal antibody (Biomedica) and immunohistochemistry for formalin-fixed and paraffin-embedded (P-IHC) tissue samples. Since only little information is available on the best technique to stain C4d, we compared the two methods most often used for detecting C4d in renal allograft specimens.
